Some people wish to decide for themselves before a medical crisis occurs exactly what medical care they will receive in the event they are faced with a severe medical crisis (i.e. debilitating stroke, cardiac arrest, severe brain injury/comma, a terminal illness).
The State of Ohio has enacted legislation addressing a person's right to pre-plan what they want done in this event. A person wishing to decide for him/herself what will and will not be done in a medical crisis may be documented by completing a, DNR Comfort Care Identification Form.
In order to assist in your completion of the form, below are some points to consider: The most important thing you need to know is that you must have your doctor's signature on this form, or it is not legal for emergency medical personnel or hospital personnel!
You need to make sure you mark the correct block that indicates your wishes on the form:
- DNRCC - By marking this block, a person is indicating they want very limited emergency assistance by medical personnel. Please see Page 2 of the form which explains what care you will receive.
- DNRCC-Arrest - By marking this block, a person is indicating they want all medical assistance available and appropriate for their emergency situation up to the point where their heart stops beating or they stop breathing.
Turtlecreek Township Emergency Life Squad personnel cannot abide by other forms of advanced orders - living wills, etc.
We must be presented with a signed copy of the official DNR Comfort Care Identification Form in order to withhold resuscitative efforts (CPR) should we be called to your home due to a medical emergency AND you, or your loved one, have already stopped breathing when we arrive. If you (or someone at the scene) cannot present the DNR, then we are legally bound to begin CPR and transport to the nearest emergency room.
If you complete this form, please store it in a place that is easily accessible at your residence so it can be given to life squad personnel when they arrive.
